School Test Coordinators are encouraged to use the Online Accommodation Verification Form to submit accommodation requests. This Google form automatically uploads to the Assessment Section office and is available here. School TCs must use their HI DOE Gmail account to access and submit this electronic form. The deadline to submit accommodations for the 2019-20 school year is January 17, 2020.
